Apollo Behavioral Changes Legal Protection Agreement

This agreement (“Agreement”) is between you, the client, and Apollo Behavioral Changes, a personal life coaching service. The purpose of this Agreement is to inform you of the terms and conditions regarding the services provided by Apollo Behavioral Changes and to protect both you and Apollo Behavioral Changes from any legal action.

Important Notice: Apollo Behavioral Changes is not a medical facility and is not licensed to provide medical advice. Our services are not intended to diagnose, treat, or cure any physical or mental health condition. The services provided by Apollo Behavioral Changes are intended for educational and informational purposes only and are not a substitute for professional medical or psychiatric advice.

By signing this Agreement, you understand and acknowledge that:

1. Apollo Behavioral Changes and its affiliates are not responsible for any negative health effects that may occur as a result of the advice given during the coaching sessions.
2. You release Apollo Behavioral Changes and its affiliates from any and all legal action that may arise from the services provided by Apollo Behavioral Changes.
3. You are fully responsible for your own health and well-being and acknowledge that it is your responsibility to seek medical or psychiatric advice if needed.
4. You understand that the information and advice provided during the coaching sessions are not a guarantee of success and that the results may vary from person to person.
5. You agree to follow all of the advice and recommendations are given during the coaching sessions at your own risk.
6. You understand that the coaching sessions are confidential and that Apollo Behavioral Changes will not disclose any information about your sessions to any third party without your written consent.
7. You acknowledge that the services provided by Apollo Behavioral Changes are intended for educational and informational purposes only and are not a substitute for professional medical or psychiatric advice.

By signing this Agreement, you agree to the terms and conditions outlined above and release Apollo Behavioral Changes and its affiliates from any and all legal action that may arise from the services provided by Apollo Behavioral Changes.
